Back to School Info From Your School Nurse

If your student has a medical need, make sure Nurse Health Services is part of your back to school checklist.
If your student:
Needs to take a medication during the school day – please fill out the Medication Authorization Form found at THIS LINK
Needs to use medication such as an inhaler to manage asthma during the school day fill out the Medical Authorization for Asthma Management at School form found at THIS LINK
May require medication at school – such as an epi pen in case of accidental exposure to an allergen fill out the Medication Authorization Form found at THIS LINK
Has had a change in health since last year that may impact their ability to participate in some activities – make sure you update their health history form in their Annual Student Renewal Forms on Skyward Family Access found at THIS LINK or email Nurse Health Services at THIS LINK
Has a doctor’s order modifying any part of their school day – make sure you update their health history form in their Annual Student Renewal Forms on Skyward Family Access found at THIS LINK or email Nurse Health Services at THIS LINK
Medical authorization forms must be renewed every year. We recommend you submit your forms as soon as possible so that our staff is ready to help your student start the year off right.
