Chehalis School District students will once again have the opportunity to receive dental care at school through Valley View Health Center dental clinics coming up April 14 at James Lintott Elementary (grade K-2), April 15 at Orin Smith Elementary (grades 3-5), April 17 at Chehalis Middle School (grades 6-8) and April 18 at W.F. West High School (grades 9-12).

You can register my March 31 using the QR code on the English or Spanish versions of the poster below. If you choose to turn in a paper copy of the permission slip for your student, please do so by March 28.
